The MGH Nephrology Division offers a wide range of comprehensive and high-quality services to the patient with renal disease, through the in-patient, out-patient and TeleMedicine settings. These are delivered by a dedicated group of physicians, using state-of-the-art diagnostic techniques and treatment protocols. The consultative, dialysis and transplant services are coordinated through an End Stage Renal Disease (ESRD) Program, a multidisciplinary program staffed by members of the Renal and Transplant Units. The mission of the ESRD Program is to provide a comprehensive and cohesive quality care to the patient with renal disease, from its earliest manifestations to renal transplantation if necessary. The ESRD Program also emphasizes preventative care (through its Early Renal Failure clinic and the Center for Renal Education), and scientific synergy and collaboration among the Program's basic and clinical science investigators in the fields of immunology and transplantation biology. The outpatient services are provided through Renal Associates, a new renal and general medicine practice, located at the main campus in the Bigelow building, Suite 1003.

In-patient Services

Consultative Nephrology

The Renal Division provides comprehensive in-patient consultative services at the Massachusetts General Hospital, Massachusetts Eye and Ear Infirmary, and Spaulding Rehabilitation Hospital. Consultations range from acid-base, water and electrolyte abnormalities to acute or chronic renal failure.

Dialysis Services

The dialysis program provides all the necessary services required for the optimal management of the patient with acute renal failure or end stage renal disease (ESRD). Specialized services include the placement of vascular or peritoneal dialysis catheters for the conduct of therapeutic hemodialysis, hemoperfusion, peritoneal dialysis, or hemofiltration. The experienced physician and nursing staff of the state-of-the-art in-patient hemodialsyis unit deliver, in conjunction with the Intensive or Critical Care unit personnel, the optimal medical support for the patient with acute loss of renal function. Patients with end stage renal disease requiring continuous replacement of renal function are initiated on hemodialysis or on continuous ambulatory peritoneal dialysis. Vascular and Transplant surgeons, nursing, social workers and nutrition teams assist the nephrologist in providing the long-term comprehensive care of the patient with ESRD.

Renal Transplant Service

The Renal Transplantation Service at MGH is a major national transplant center offering children and adults, renal and simultaneous renal -pancreas transplants. The MGH has been designated a Center of Excellence in Transplantation by Medicare and other leading private insurance carriers. A multidisciplinary team of senior transplant specialists provide comprehensive surgical, medical and psychological care, and state-of-the-art approaches to immunosuppression and infectious complications.

High Blood Pressure Clinic

The focus of this Clinic is to offer education and treatment for patients with high blood pressure of various causes. The Clinic also has a special interest in the management of young women with high blood pressure who are either considering pregnancy or who experienced high blood pressure in pregnancy. Patients are evaluated about possible causes, acquired or inherited, and are educated regarding all current forms of drug therapy. Social services and dietary support are included in this multidisciplinary approach.

Kidney Stone Clinic

The MGH Kidney Stone Clinic focuses on the prevention of new kidney stone formation and the treatment of existing kidney stones. The medical and surgical approaches to stone treatment are highly integrated and complementary due to the close working relationship of the nephrology and urology physicians. The most advanced surgical techniques and instruments are available for the removal of existing stones. Patients are evaluated to determine the cause of their kidney stone and then are prescribed the optimal preventive therapies which may include dietary modification or medication. Research into the causes of stone formation and stone prevention are an active part of the clinic and of a very high priority.

Nephritis and Vasculitis Clinic

In this clinic, the patient with lupus or other forms of nephritis or vasculitis is longitudinally managed, and treated. Management involves a general evaluation of renal function using several studies that include renal imaging, diagnostic serology, and percutaneous renal biopsy. Treatment options are discussed and relevant literature provided. Immunosuppressive and other forms of treatments are tailored to the patient's specific needs.

Interventional Nephrology Clinic

This clinic offers complete evaluation and consultation for hemodialysis patients with complicated hemodialysis vascular access problems. The clinic also offers consultation for patients with renal artery stenosis, as well as peripheral arterial diseases related to vascular access.  The pre-hemodialysis or hemodialysis patients with problematic hemodialysis fistulas or grafts are evaluated before further surgical and endovascular interventions.

The MGH Center for Renal Education provides a support service for patients and families. In collaboration with MGH nephrologists, experienced renal nurses, a dietician, and a social worker, the Center provides education about chronic renal disease and its management and individualized plans of care that include nutritional counseling, blood pressure management, medication review and supportive services. The health team is dedicated to help patients live well with renal disease. Information concerning renal replacement options including hemodialysis, peritoneal dialysis and transplantation is offered as well. The Center has received the 2000 Partners in Excellence Award for its leadership and Innovation and quality treatment and service.

Continuous Ambulatory Peritoneal Dialysis

The Continuous Ambulatory Peritoneal Dialysis program is a chronic out-patient service for home peritoneal dialysis patients. The philosophy here is to provide a collaborative environment in which patients and families can be trained, supported and followed. This service is staffed by MGH nephrologists, renal nurses, a dietitian and a social worker who provide the support needed to the home peritoneal dialysis patient. Additional services provided are interventions for dialysis-related issues and in-patient consultations.

A multidisciplinary team of transplant physicians and surgeons provide the evaluation and selection of potential candidates and donors, post operative care, immunosuppressive management, and psychological and social support. Innovative approaches to immunosuppression as well as the prevention and treatment of infection in the transplant recipient have been pioneered at this institution.

Transplant Recipient Evaluation

All candidates are interviewed, the option for kidney transplantation including the risks and benefits are discussed and a full review of the candidate's history, physical examination and laboratory tests including tissue typing are performed. Nurse coordinators and social workers as well as the nephrologist, surgeon and psychiatrist play an active role in the recipient evaluation. A definitive plan regarding candidacy or need for further tests is made at the time of the Family Meeting, where the candidate meets with the entire Transplant Team.

Transplant Donor Evaluation

Prospective kidney transplant donors- Both related and unrelated donors are thoroughly screened by MGH staff nephrologists before they are accepted as donors. Potential donors have the opportunity to consider whether they wish to go forward with donation in an atmosphere of total confidentiality. In the appropriate anatomic situation, a laporoscopic surgical approach may be offered to the donor.
